Where does my data go when Royal Forms™ is submitted?
Your data is sent to our secure Royal servers, which is equipped with full security,
compliance, redundancy and disaster recovery. It is safer and less expensive than
housing the data locally, at your place of business.
Next, a copy of the Royal Forms™ is transmitted via encrypted email to an email address
of your own determination.

Can Royal Forms™ automatically submit into our systems?
Yes, options are available that allow the submitted Form data to be sent to your
electronic health system automatically. This type of integration will create several

Can we house the data on our own local server?
Yes, an option to house form information on your server is always available, though
this option requires more setup and falls outside the standard Royal Online Form
pricing.

Can we brand the landing page after Form submission?
Yes! After a Form is submitted the user is directed to a web page. This web page
can have your logo, exam prep instructions, directions to your facilities and printing
capabilities.

How much do Royal Forms™ cost?
$100 a form per month plus a one-time setup fee of $1,250 per form. Volume discounts
are available.

How do referring physicians access the data?
There is an encrypted email trail for every submission. Also, all of the data is
available by accessing the Royal portal. At the physician's discretion, Royal Forms™
data may automatically feed to an EMR system.

How is my data protected?
Your data is housed in a HIPAA compliant environment where access is limited to
only Royal products. When Royal sends or receives your data, it travels over 256-bit
SSL connections and all patient health information carries a 2nd level of encryption
to prevent intrusion. Encryption is achieved using AES 256-bit government level
security.

Can Royal passwords match our system's passwords?
Yes, we allow you to automatically feed all usernames and passwords into our systems
so that your end-users do not need to remember a new password. You can update passwords
at any time you wish. We recommended that passwords are updated every 90 to 120
days.

Will Royal conduct the Form modifications that we want?
Yes. If you need to modify a form, feel free to ask us anytime. We will complete
any small change to a form, free of charge.
